Appointments are available every 15 minutes during our office hours which are:
Monday, Tuesday & Wednesday:  9 am to Noon, and 2 pm to 6 pm
Friday:  9 am to Noon, and 2 pm to 4 pm.
(Same day appointments are available -- available by phone only.)

We are open Monday to Wednesday and Friday by appointment only.  The initial consultation and examination takes about one hour.  Our Initial Health History Form is available online.  Print it out, fill it out at your convenience and bring it with you to your first visit.  In addition, please bring all necessary insurance and injury information.
